What if my insurance or demographics have changed since my last visit?

It is the patient’s responsibility to communicate any changes in their healthcare information to the front desk upon their arrival.   This information may include:

  • Insurance information/coverage
  • Responsible financial party
  • Address, Phone, Email
  • Emergency Contact
  • Change of referring and/or primary care physician
