- Please use your patient’s FULL LEGAL NAME as is appears on their insurance card along with the patient’s email address when scheduling their online appointment. The appointment confirmation will be sent to the patient’s email address that has been given.
- Please enter the patient’s Medical Insurance Plan name in the Insurance plan field when scheduling the online appointment.
- Only appointments that are available in the next 90 days will appear. If you are requesting an appointment after the 90 days shown, please call our office at (402) 484-9000 or 1-800-742-2224.
- To assist our doctors, please fax your exam notes, photos and testing prior to your patient’s appointment to 402-483-4223.
- Patients age 18 or younger need to have a parent, legal guardian or approved non-parent guardian present at their appointment. A parent or legal guardian must attend the initial appointment to sign all paperwork.
